SIU School of Medicine — “Randy”
CURRENT Level B + customer-reported- Organization
- Southern Illinois University School of Medicine
- Industry
- Academics / healthcare
- Agent
- Randy Rhodes; 11 AI patients reported live by the case study
- Problem
- Game-based simulation could not handle unpredictable student questions and could not reproduce the human face-to-face trust-building dimension of clinical interviews.
- Why D-ID
- SIU needed realistic, responsive visual patients rather than a text-only chatbot.
- Implementation
- D-ID Real-Time embedded agents provide the visual layer; SIU built patient personas and backend integrations and used a custom LLM/RAG setup. ElevenLabs supplied customized voices.
- Human work
- Prompt engineering, patient-persona design, backend integration, scenario testing, anonymized feedback collection and academic validation.
- AI work
- Real-time dialogue, adaptive symptoms/conditions/emotional states, avatar performance and repeatable patient simulation.
- Outcome
- Students can practice on phones, repeatedly and at their own pace; a 10-university alpha was launched and millions of interactions were expected. No clinical-outcome improvement is claimed here.
Before vs after
| Metric | Before | After | Evidence |
|---|---|---|---|
| Simulation format | Game-oriented approach was inadequate for unpredictable dialogue | Real-time AI patients | Customer/D-ID case |
| Repeatability | Limited by traditional simulation | Students can practice as many times as needed | Case study |
| AI patient count | — | 11 live; more released weekly at time of case | Company-reported |
| Clinical outcomes | — | Not publicly disclosed | Evidence boundary |
Safety & evidence: This is a training simulation, not a clinical diagnostic or treatment system. The case describes anonymized feedback and planned academic validation. No patient-health outcome claim is made.

Historical D-ID AI Projects & Product Demonstrations
chat.D-ID
HISTORICAL PRODUCT / DEMO Not a customer casechat.D-ID was a web app combining D-ID's real-time text-to-video streaming technology with OpenAI's ChatGPT so users could converse face-to-face with a photorealistic AI avatar. It was launched in beta and presented as a demonstration of D-ID's streaming technology.
Current status: The 2023 launch should not be treated as evidence that chat.D-ID remains a current standalone product. Current D-ID materials instead emphasize Visual Agents and V4 Expressive Visual Agents.
Source: D-ID launch announcement.
D-ID Chat API / Real-Time Digital Humans
HISTORICAL PRODUCT LAUNCH Not a customer resultD-ID launched a real-time streaming/chat API for face-to-face conversations with AI digital humans. The announcement described integration with LLMs such as GPT-3 and LaMDA and potential uses in customer support, sales, training and personal finance.
Evidence boundary: These were product capabilities and intended use cases. They are not customer outcome claims. Current implementation details should be checked against D-ID's current API documentation rather than assuming the 2023 architecture remains unchanged.

|---|---|---|---|
| Average CTR | 16.3% | 11.6% | +4.7 percentage points |
| Email clicks | Personalized | Non-personalized | 18% more clicks |
| Coupon redemptions | 1,812 | Non-personalized comparison | 30% higher redemption rate |
| Build time | ~1 hour per email | — | Bulk of work automated |
Classification: This is not a customer case. It is a D-ID pilot using its own marketing technology, so the findings should be treated as company-reported internal evidence.

D-ID describes knowledge, memory, a digital-human interface and agentic capabilities, plus analytics such as conversation volume, engagement time, question topics and sentiment.
Evidence boundary: The launch demonstrates a product capability. It should not be converted into claims about customer performance until individual deployments disclose results.
Source: D-ID launch announcement.
V4 Expressive Visual Agents
CURRENT PRODUCT Capability, not customer resultD-ID launched V4 Expressive Visual Agents for real-time, LLM-connected conversations. D-ID reports sub-0.5-second conversational turns, up to 4K output, sentiment-aware facial expression, optional camera/vision, inline media/UI and MCP-app integrations. D-ID's technical page reports a June 2026 SyncNet benchmark and other performance figures; those are vendor-reported product benchmarks, not customer outcomes.

Safety & Responsible AI
Healthcare
SIU's Randy is a training simulation, while AFA's Allison is an information/support assistant. Neither is treated here as evidence of autonomous diagnosis or treatment. Human escalation is explicitly part of the AFA workflow.
Identity & likeness
Digital twins and cloned voices require appropriate consent, rights and disclosure. Envy used a disclosed digital twin; the SOMOS project used a family member's likeness/voice context. Exact legal-consent terms are not assumed where sources do not state them.
Defense
Rafael is documented only at the public level of technical product explanation. No inference about classified or operational capabilities is made.
Grounding
D-ID's current agent architecture describes customer-controlled knowledge/RAG and optional controls, but individual case pages do not disclose every guardrail. Do not transfer platform-level governance claims automatically to every deployment.
Human review
Across the documented cases, humans remain responsible for scripts, content strategy, persona design, knowledge preparation, editorial review, campaign decisions or professional escalation.
Deepfake risk
Transparent disclosure matters for digital twins, historical figures and voice/likeness cloning. Envy's case explicitly recommends being transparent that the avatar is an avatar.
Evidence-Based Patterns & Researcher Inference
Evidence-based observations
- Training is a recurring deployment area. SIU, Diplomat and SPIN use avatars to make training or learning more repeatable and scalable.
- Marketing use is often about production throughput and personalization. Envy, Pitango and Gameloft use avatars to increase the amount or personalization of content without repeating physical shoots.
- Visual Agents move the avatar from content to interface. SIU, Gatorade, HARTMANN, Rafael and CreatorUp use the avatar as an interactive front end to knowledge rather than only as a presenter.
- Quantitative customer metrics remain uneven. Envy, My Last Will, SingIt, SPIN and Hakuhodo disclose concrete measurements; several current Visual Agent examples do not.
- Third-party technology is common. SIU, AFA and SOMOS show that D-ID is often one layer in a larger stack involving LLMs, RAG, voice systems, messaging channels or custom applications.
- Human work is not eliminated. Scripts, knowledge curation, persona design, editorial decisions, testing and escalation remain important across the cases.
Researcher inference — clearly separated
- Avatars appear most useful where the human face is part of the job. The documented examples suggest a stronger fit for tutoring, simulated patients, product explanation, prospect communication and emotionally sensitive guidance than for tasks where text alone is sufficient.
- The measurable value tends to come from workflow redesign, not the avatar alone. Envy's results combine personalization and a different outreach process; SIU combines D-ID with a custom LLM/RAG system; AFA combines D-ID with human escalation.
- Visual Agents create a new measurement problem. For interactive agents, conventional video views are less informative than conversation completion, question topics, escalation, lead creation, satisfaction and downstream actions. D-ID advertises such analytics, but public customer-level numbers are still limited.
Source Register
The research prioritizes D-ID primary material, customer-owned project pages and credible independent/award sources. D-ID pages are explicitly labelled as company-reported where independent verification is absent.

Artificial Intelligence Files — Editorial Conclusion
The documented D-ID cases show a progression from AI-generated presenters toward interactive digital humans. The strongest evidence is not that an avatar automatically improves business performance; rather, the cases show organizations redesigning specific workflows around repeatable video production, personalized communication, simulation, knowledge access and real-time interaction.
Where metrics exist, they are presented with their source and limitations. Where a customer or D-ID does not disclose a number, this database does not estimate it. Product capabilities such as lead qualification, booking, analytics or real-time conversation are not treated as customer results unless a deployment documents those outcomes.
